4 to 6 Hour Block

$4,000 Flat

Up to 6-patients

6 to 8 Hour Block

$5,200 Flat (800/hr thereafter)

Up to 10-patients

"Billable Anesthesia Time"

Starts when an anesthetic agent (IV, IM, Inhalational, or Local) is administered on the first patient of the day, and

Ends when the dental team both 1) completes the final procedure of the day, and 2) no longer comes in physical contact with the patient

Payments

The anesthesia fees are billable to the dental office (not the patient) at the end of the day

Payment Method: Check payable to "Han Anesthesiology"

A W-9 form will be provided to the dental office upon request

Fees are flat rates, and not pro-rated

* If needed, we can provide suggestions on how to coordinate payments with patients, but ultimately the payment terms and conditions are between the dental office and the patient
